To redeem referral card below:

Both patients must receive treatment at office to be eligible for redemption. Please allow us up to 14 business days to process your request. You will receive your reward in the mail.

 

CONTEST

The Rules: 

 

1.) Patient must wear their SmileKrafters T-Shirt to their appointment

    (this is the number 1 rule if they aren't wearing the Tshirt then they dont qualify)

 

2.) Must be on time or early for their appointment.

 

3.) Must have good Hygiene - home care.

 

4.) NO broken brackets or wires. 

 

A Token will be given if all the rules have been followed. When the patient has received 3 Tokens they can hand them in to the assistant and they will put their name on a raffle ticket. 

 

At the end of the 3 months a raffle ticket will be drawn and that patients name is the winner.
